How does the referral process work?
The referral process is very easy, and Unity's staff is happy to answer
your questions and guide any necessary paperwork.
A referral begins with a phone call to Unity
recommending an individual for either hospice or palliative care. Our
administrative office hours are 7am to 5pm, Monday through Friday; however,
Unity accepts referrals 24 hours a day, 7 days a week, through our after-hours
call staff by calling Unity's main phone number.
After receiving a referral, Unity contacts the prospective patient,
representative or health care agent under an activated Power of Attorney for
Health Care, if appropriate, to discuss their interests in either hospice or
palliative care. Our goal is to make this contact within 12 to 24 hours of
receiving the referral. Individuals are at no time obligated to sign onto
a Unity program, and, as always, Unity upholds strict confidentiality measures
during the inquiry process.
If the prospective patient wishes to consider hospice or palliative care with
Unity, we then contact his or her physician and assess eligibility. The
final step in the referral process is arranging for a meeting with the
prospective patient and family to discuss care needs and how Unity can help.
