Community Resources
                 and Assistance Programs

A variety of programs are available to support the residents of Northeast Wisconsin.  While not all-inclusive of the resources available, the following list provides suggestions for support for you and your loved ones.  For Unity patients and families, consult your social worker for additional information and assistance with the following resources.

Adult Day Care
Some counties offer various adult day care programs.  These facilities care for patients during the business day.  Transportation and the cost of the day care are the responsibility of the patient.  For more information on adult day cares, call the senior center or human services department in your county.

Community-Based Residential Facilities (CBRF)
CBRFs provide a family-like living environment for persons who cannot remain at home, but are still somewhat independent.  The cost for room and board is the responsibility of the patient, but Unity can continue care in any contracted CBRF.

Community Options Programs
This is a county program that offers a variety of services/funds to families who have a medical and financial need.  You may find out more information by calling your county human services department.

Crisis Centers
Most counties offer a crisis intervention service which will connect you with a counselor 24-hours a day to help with non-medical crisis situations.  Consult a telephone directory for phone numbers in your area.

Funeral Homes
The telephone directory yellow pages will provide you with a complete listing of funeral homes in your area.  The funeral home staff will be a great resource to you any time you are ready to begin discussing your plans.

Handicapped Parking Permits
Permit applications are available at the local Division of Motor Vehicles or your physician's office.  The application requires your physician's signature.

Lifeline
The Lifeline program is available in each county for persons who want to maintain their independence by living alone but want additional safety and security.  The purpose of the Lifeline program is to link its users to 24-hour assistance through a personal help button that the patient wears.  Contact Lifeline at 800.380.3111.  Alternately, contact St. Vincent Hospital at 920.433.0111 or another local hospital to learn more.

Medicaid (Medical Assistance)
This is a government medical insurance program for persons who meet the medical and financial criteria.  Medicaid may reimburse for medications, medical supplies and nursing home care for eligible persons.  Applications can be obtained from the human services department in your county.

Mobile Meals
Most counties offer a home-delivered meal program for people who are unable to manage their own meal preparation.  Mobile meals are often available through the county senior center or local hospital.

Nursing Homes
Unity provides care in most of the nursing homes in your community.  Patients are responsible for payment of room and board.  Medicare pays for room and board in limited cases, such as those patients who are admitted to the nursing home for rehabilitation and have had a recent rehabilitative hospital stay.  Medicaid is available to pay for nursing home room and board for patients who meet the financial eligibility criteria.  If you are considering nursing home placement, we encourage you to tour the facility you are interested in and get your questions answered by that specific nursing home staff.

Private Caregivers
You have a variety of options available to you if you are considering hiring private caregivers to help in the home.  The cost for hired caregivers who provide custodial care is the responsibility of the patient/family.  Some senior centers offer caregiving programs.  You may check the telephone directory yellow pages for a complete listing of programs in your area.  Try searching under "Home Health Services," "Senior Centers" or "Aging Resource Centers."

Senior Centers
Most counties have information and resources available for their seniors.  These agencies may be listed as senior center, aging resource centers or commission on aging.  Call your county program for further information.

Transportation
There are many companies who provide medical transport.  The cost is based on mileage.  Some companies require payment up front.  Consult the "Transportation" section in the yellow pages of your telephone directory for medical transport companies in your area.
