with access to around-the-clock comforts and care provided by Unity's

Who is eligible?
Individuals who are in the final stages of life and would like to receive
skilled nursing care in an alternative, home-like setting.
Who may refer a patient?
Referrals may be provided by a physician or health care professional, clergy,
family member, friend or the person seeking care. To learn more or to make

What is the cost and how is it paid for?
Unity is able to offer room and board at a low cost to our patients. Daily
room and board are covered by a patient's private insurance, private pay and
community support; hospice care services are covered by Medicare, Medicaid and
other funding sources.
What is the difference between Unity's
hospice residence and a hospital or nursing home?
Unity's Residence is a short-term care facility that offers a home-like
environment staffed with end-of-life care specialists. Hospitals provide
an acute medical care setting and do not offer residential care. Nursing
homes generally provide long-term skilled care.
When can loved ones visit?
The Residence welcomes family and friends 24 hours a day, 7 days a week.
The private patient rooms cater to guest visits and include a fold-out bed to
accommodate overnight stays by a family member. Planned pet visits may
also be arranged. Residence amenities are available for guest use.
