First hospice in the state of Wisconsin and the largest hospice program in Northeast Wisconsin, since beginning in 1977. In 2002, Unity became the first hospice organization in Wisconsin to offer a palliative care program in both the in-patient and home settings.

Serves all or parts of 12 counties in Northeast Wisconsin:  Brown, Calumet, Door, Forest, Kewaunee, Manitowoc, Marinette, Menominee, Oconto, Outagamie, Shawano and Waupaca.

Locally owned by Bellin Health, St. Mary’s and St. Vincent Hospitals, Unity operates as a financially-independent 501(c)(3) not-for-profit organization.

Unity’s Green Bay-area campus is located on 34 acres in the town of Ledgeview at 2366 Oak Ridge Circle, De Pere, WI  54115.  Additional staff workstations are located in Marinette, Shawano and Sturgeon Bay.

Services provided:

    • Hospice.  Comfort care for people with a six-month-or-less prognosis who are no longer receiving curative treatment.
    • Palliative.  Comfort care for people with a life-limiting illness, including those with a less-than-six-month prognosis, and still receiving curative treatment.
    • Residential.  The Jack and Engrid Meng Residence offers end-of-life care to hospice patients who are near their final stage of life.
    • Bereavement.  Grief support services provided to children and adults through individual and group counseling, special programs, newsletters and other mailings.

Provides care for over 800 patients per day on hospice and palliative programs and over 5,000 family members and friends through bereavement services.

In 2010, Unity cared for a total of 2,245 hospice and palliative patients.  An additional 6,558 people participated in Unity’s bereavement program for the same period.  Of the patients served by Unity, two-thirds were cared for in a private home and almost one-third were cared for in a nursing home or assisted living facility.  Unity partners with over 100 nursing homes and assisted living facilities in Northeast Wisconsin as well as seven hospitals.

Unity’s Jack and Engrid Meng Residence, the first hospice residence in Unity’s Northeast Wisconsin service area, opened in July 2007.  The Residence offers end-of-life care to hospice patients who are near their final stage of life.  In 2010, the Residence served 143 patients.  Two rooms are designated for patients in need of community support to cover the cost of room and board.  Residence features include:

    • Around-the-clock comforts and care provided by skilled staff.
    • 12 private rooms, each room is fully furnished with a bathroom and shower, TV/DVD, telephone, linens and towels, and individual temperature controls for heating and cooling.
    • Full kitchen, living room and two family rooms, including a special children’s area, are available to patients and guests.
    • Large patio, ponds, gardens and walking path.
    • Handicap-accessible building.

Gardens are being developed at Unity’s Green Bay-area campus.  Funds raised through these gardens help support the care of underinsured and uninsured patients. Charity care in 2010 totaled nearly $1.4 million.

Unity is a member of the National Hospice and Palliative Care Organization (NHPCO) and the Hospice Organization and Palliative Experts (HOPE) of Wisconsin.
