Who is eligible?
Individuals who are in the final stages of life and would like to receive
skilled nursing care in an alternative, home-like setting.
What is provided?
● 24-hour skilled nursing
care

Assistance with medications, bathing and other special services
● Individual care plan managed by personal care team (including
skilled nurses, physicians, certified nursing
assistants, social workers,
grief counselors, chaplains and volunteers)
●
Medical equipment, supplies and medications based on individual
needs
●
Prepared meals and snacks
●
Laundry service on-site for machine-washable clothing

Housekeeping
Where are the services offered?
The Jack and Engrid Meng Residence, which features 12 private rooms and is
situated on Unity's beautiful, 34-acre campus in De Pere, Wisconsin.
When is care provided?
24 hours a day, 7 days a week
How is it paid for?
Unity is able to offer room and board at the Residence at a low cost to our
patients. Daily room and board are covered by a patient's private
insurance, private pay and community support; hospice care services are covered
by Medicare, Medicaid and other funding sources.
