Hospice
Hospice is a specialty that provides comfort care to people who are in the last
six months of their lives and are no longer seeking or receiving curative
treatment.
Who is eligible?
● Patient of any age with a
life-limiting disease or condition
● Patient who is expected to
have a prognosis of six months or less if
the disease or condition takes its expected
course
● Patient who is no longer
seeking or receiving curative treatment
What is provided?
● An interdisciplinary
team that supports the physical, spiritual and
emotional needs of the patient and family,
including:
- Patient's
physician
- Medical
director
- Skilled
nurse
- Social
worker
- Certified
nursing assistant
- Grief
counselor
- Chaplain
- Volunteers

People services, medications, supplies and equipment
●
Care plan based on patient's individual needs and determined in
collaboration with patient's primary care team
●
Option to receive care at Unity's Jack and Engrid Meng Residence
● Bereavement care for
patient's family and friends
Where are services offered?
Hospice services are offered wherever the patient resides, and patients are not
required to leave their homes. Services may be offered in the patient's
private home, nursing home, community-based residential facility (CBRF),
residential care apartment complex (RCAC), hospital or Unity's Jack and Engrid
Meng Residence.
When is care provided?
24 hours a day, 7 days a week
