Unity's Care Team
Each patient and family on a Unity program
works together with a personal care team to
create their individualized plan of care.  Unity's care teams consist of nurses, social workers, certified nursing assistants, volunteers, chaplains, grief counselors and our medical director.

We also collaborate with our patients' existing physicians and other medical professionals to create a seamless care experience for those on our hospice and palliative programs.
 
Meet our patient care staff ...
w Skilled Nurses provide expertise in managing patients' physical symptoms and support patients whenever care is needed.
   
w Social Workers provide emotional support and assist patients and their families in obtaining community and financial resources.
   
w Certified Nursing Assistants care for patients in a variety of ways, including with personal cares and bath assistance, depending on the patient's individualized plan of care.
   
w Trained Volunteers support various patient needs, including companion-sitting, light meal preparation, light housekeeping, transportation, delivery of supplies, assistance with personal cares, spiritual support and bereavement support.
   
w Chaplains can serve as a spiritual provider to patients and families of all religious or non-religious backgrounds as they work through end-of-life feelings and concerns and/or can act as a liaison to the patient's chosen faith community.
   
w Grief Counselors offer bereavement support to patients' loved ones for minimally 13 months after services end and are trained to talk about issues of grief and loss.
   
w Unity's Medical Director, Rance Hafner, MD, oversees all medical care provided by Unity staff and is also available for patient consultations when requested.
